Life and Dental insurance can be added to your medical coverage. They can also be purchased separately if you have at least 10 employees (25 for dental only).
The amounts of life insurance are $20,000 - 30,000 - 40,000 or 50,000 on each employee. Your company must pay 100% of the cost and all employees must enroll. The cost is 20 cents per $1000 of coverage. Accident Death coverage can be added for an additional 3.5 cents per $1000.
There are 3 dental plans to choose from offering a choice of 1) network providers only, 2) use any dentist but payment is at the negotiated rate, or 3) use any dentist but non-network providers will cost you more. Rates start as low as $19.32 an employee and
$63.68 for family coverage.
